To qualify for a FIDE Master (FM) title a player must achieve a FIDE Rating of 2300 or more. Candidate Master (CM) is similar to FM, but with a FIDE Rating of at least 2200. All the titles listed above are open to men and women.

It is by far mostly made. Chess is no exception to hard work paying off. Every single professional has spent the majority of their life studying the game, equivalent to tens of thousands of hours of pure work. If I were to quantify this, I would estimate that it is something like 90% made, and 10% born.
